SoCal Hammers Participation Waiver

I, ____________________________________(Guardian), on behalf of myself, members of my family, my heirs, executors, administrators and assigns, hereby forever release, discharge and hold harmless So Cal Hammers located at 2731 North Grand Avenue Santa Ana Ca 92705, its administrators, directors, members, volunteers, employees, other participants, sponsors, owners, and So Cal Hammers from any and all liability claims, demands, actions and causes of actions whatsoever arising out of or related to any loss, damage, or injury including death, that may be sustained by me, or to any property belonging to me, while participating in sports performance and related athletic activities. I agree to indemnify and hold So Cal Hammers harmless and release them from any and all liability for any injury which may be suffered by _____________________________________ (Youth Athlete / Participant), arising out of or in any way connected with participation in So Cal Hammers.  

I fully understand all risks associated with wrestling/sports performance training and all related athletic activity. I fully understand that I assume all risks for any injuries received and agree to accept financial responsibility.  By signing this waiver I state that my child is of good health and has been cleared for physical activity by a licensed physician. In the event that your child is currently prescribed any medication please consult the prescribing physician to insure that physical exertion does not exacerbate the health condition that the aforementioned medication is prescribed for.

I also understand that So Cal Hammers Wrestling Club may use my picture, and/or any video for advertising purposes. 
In signing this release, I acknowledge and represent that I am at lease 18 years of age and fully competent; have read this agreement and fully understand its terms, understand that I have given up substantial rights by signing it, have signed it freely and without any inducement or assurance of any nature, and I intend it to be a complete and unconditional release of all liability to the greatest extent allowed by the laws of California.
